MIHS Football Registration 2018


To be eligible to be on the field August 15th, you must have a current physical on file at the athletic office as well as turned in the completed sports packet and your registration must be completed online. Registration for Fall Sports, opens August 1st, 2018.  Fall Registration packets for football are due by August 13th. They need to be turned into the High School Athletic Office, in order to participate in the first day of football practice, which is August 15th.  

Everyone must complete a new registration packet for the football season for 2018-2019. The packets from spring/summer will NOT carry over to the new school year.

This year for the 2018-2019 registration again is a 2 step process, online as well as returning forms to the high school Athletic Office. You will use your Skyward Family Access to login and password, OR use the students ID number(minus the leading zero) and last name as username and password.Use the below link to complete the online sports registration.

STEP 1: Online Registration

STEP 2: You will be required to submit the following forms to the Athletic Office: 2018-2019 Sports Registration Packet

1.   Athletic Health Form, which includes the Physical Exam, signed by a physician(some may have already submitted this in order to participate in summer conditioning.) You can check with the athletic office, for the expiration date of the sports physical. 

2.   Signature Page

3.   Emergency Medical Authorization Form

FOOTBALL sports packets are due by August 13th.

  • Every player needs to have an updated physical on file with the athletic office in order to play football. Summer is a great time to schedule a physical.  

The first day of Fall sports is August 20th, however, Fall football starts August 15th.

  • ALL Students participating in a fall sport must complete a sports eligibility packet and have a current physical on file.

The Athletic Office is closed the entire month of July, we will reopening August 2nd at 7:00am.

You will be required to hand in the following paper forms to the Athletic Office (NOTE - due to the construction-related school closure, please either email Becky LeMaster at, fax  to 206-230-6316 or contact Becky LeMaster by phone 206-230-6345 to make arrangements for drop off): 

1.   Athletic Health Form, which includes the Physical Exam, signed by a physician

2.   Signature Page

3.   Emergency Medical Authorization Form

4.   Receipt showing you completed the online registration/payment portion 

For Repeat Sports, you will need to submit just a new  Signature Page for that specific sport along with the receipt showing you completed the online registration/payment portion.  Remember if it is a non-cut sport payment is due at time of registration, if the sport is a cut sport, payment is required once cuts have been made and your son/daughter has made the team.

NOTE: If you are transferring from a non-MISD school, please contact the Athletic Office for instructions.

Registration Process:

Please log onto

1) Click on Online Payment Box on the MIHS homepage (Lower right corner)

2) Use your Skyward Family username and Password for access

3) Select students name

4) Select "Items at all schools"

5) Select "High School"

6) Select "Mercer Island High School"

7) Select "Sports"

8) Select "2018-2019 Sports Registration and Payment"

9) Select Registration Forms

10) Select Forms #1, #2, and #3 for athletics registration.( Even though it says BUY there is not charge for these forms.)

11) Select Checkout Step 1: Additional Information and fill out the three forms. *** Very Important you must fill out all 3 forms when registering for a sport!!**

12) Select Checkout. A receipt will appear if you have registered correctly.

To Make a Payment On-Line:

(For cut sports, please do not make payment until your son or daughter has made the team) 

1) Complete Steps above, but select the appropriate sports fee.

2) Make payment
