MIHS Sports Registration Information

This school year (2021-2022) MIHS sports registration is still two parts: the online registration via Skyward plus the printed sports packet which needs to be turned in to the Athletics office or via email to Becky LeMaster. You will use your Skyward Family Access name and password to log in OR use the student's ID number (minus the leading zero) and last name as username and password.

Use the following link for the online portion of the registration: Online Sports Registration

You will also be required to submit the following forms to the Athletic Office: 2021-2022 Sports Registration Packet

  1. Athletic Health Form, which includes a Physical Exam, signed by a physician. Valid for two years. Physical Exams DO NOT carry over from IMS. Incoming 9th will need to get a new exam.

  2. Signature Page

  3. Emergency Medical Authorization Form

This process needs to be completed each season! Forms submitted for spring/summer activities do not carry over (with the possible exception of the player physical which is valid for 2 years).
